Its activity is therefore best understood as substrate-level engagement of enzymatic machinery rather than direct receptor activation.

NAD+ serves as an obligatory cofactor for the sirtuin family (notably SIRT1 and SIRT3) and for PARP enzymes involved in DNA-damage response, linking its availability to deacetylation signaling and genomic-maintenance pathways. The cellular NAD+/NADH ratio is understood to influence redox balance and mitochondrial biogenesis, with effects associated with downstream regulators such as PGC-1α and AMPK. These connections position NAD+ as a node coupling metabolic state to transcriptional and repair signaling.

NAD+ is understood to be relevant to neuronal energy metabolism and axonal maintenance, where its availability is associated with protection against metabolic stress in neural tissue. NAD+ biosynthesis (via NMNAT enzymes) is implicated in axon resilience, though direct receptor-mediated neuroactivity is not the primary described mechanism. Its CNS relevance is generally framed around supporting cellular energetics rather than acting as a conventional neurotransmitter.

NAD+ is understood to contribute to systemic metabolic regulation, including roles in glucose and lipid handling through sirtuin- and AMPK-linked pathways. NAD+-consuming enzymes such as CD38 are connected to immune-cell signaling and inflammatory tone, and declining NAD+ levels are associated with age-related metabolic and circadian changes.
